| If you are diabetic, the specialist nurse would like to see you at least once a year for a full check up/health review. You will be invited by letter on the month of your birthday to attend an Annual Health Review. You will also receive a blood request form which you should take to your local Phlebotomy clinic to have your bloods done as soon as is convenient. Angela, our Healthcare Assistant, will contact you within 4 weeks from you receiving the letter to arrange a mutually convenient appointment for you to attend the surgery. |
