If you are planning a holiday or business trip abroad you may require travel vaccinations, malaria tablets or travel advice. A travel questionnaire will need to be completed well in advance of your departure date (at least 8 weeks) The surgery will then contact you to make an appointment. Please note that a private prescription charge of £10 is payable for certain malaria tablets.

You can collect the travel questionnaire from reception or you can print it from the attachment below. (note: You will require one questionnaire per person)

Once you have handed in the questionnaire, you will be contacted by our Travel Nurse who will advise you on what you will need and to make the necessary appointments.

Yellow Fever Centre

We now operate as a Yellow Fever Centre at a cost of £45.00, which is payable at the time of vaccine. This is a one off vaccine which lasts for 10 years. Yellow Fever must be given at least a minimum of 10 days prior to travel.
